Accident Investigation Procedure

The primary focus of any accident investigation should be the determination of the facts surrounding the incident and the lessons that can be learned to prevent future similar occurrences. Accidents occur when hazards escape detection during preventive measures, such as a job or process safety analysis, when hazards are not obvious, or as the result of combination of circumstances that were difficult to foresee.

Be prepared with a policy, team, and investigation kit; Conduct a complete investigation, including how to assess the accident scene, collect evidence and interview witnesses; Conduct an analysis and find the root causes; Report and follow up. An investigation is an "after the fact" response. It is a process that uncovers hazards or problems that can be eliminated so similar events will not happen in the future.  To provide a safe and healthy working environment for all employees, each bureau of the DOI will develop and implement an annual safety and health training plan that encompasses the following.

Office of Accident Investigation

• Investigating major or significant accidents and incidents to identify safety deficiencies and unsafe conditions which are then referred to the responsible FAA office for evaluation and corrective action.
• Ensuring that the FAA fully addresses New NTSB Safety Recommendations.
• Utilizing the Accident Investigation Quality Assurance Program to improve the quality of all accident and incident investigations performed by the FAA.
• Analyzing accident and incident data as well as other safety data to identify safety issues and trends.
